Community Hospices helps enhance the quality of life for people with advanced chronic and life-limiting illnesses by providing comfort care which means relieving physical, emotional, and spiritual suffering. We provide hospice care wherever people live: in homes; long-term care facilities; group homes or hospitals. We also help manage acute symptoms and pain in our hospice Inpatient Unit co-located with our skilled nursing facility.
Hospice is:
- a concept of care for people with terminal illnesses, their caregivers and their families, that allows them to make decisions about their care
- a plan that treats the emotional and spiritual needs of patients, their families and loved ones - unlike traditional medical care that focuses only on the physical needs of patients
- a plan that enables patients with terminal illnesses to live their remaining days comfortably, surrounded by the people and the things they love
- a plan that provides terminally ill patients and their loved ones
- comfort
- control
- choices
- compassion
