STEP
ONE: Request and complete an Application
To request an application please contact Dollrinia Butler in the admissions
office by calling 202-895-0121. (If you are calling over the
weekend, please use our main number, 202-966-3720.)

To

have a copy you can download it now.
Return the application to The Washington Home. There is no application
fee at The Washington Home.

If necessary, the prospective resident is placed
on a waiting list for the unit. If so, the applicant or his or
her representative
is called when there is availability. If not, the application
process continues uninterrupted.
The applicant’s representative
will request medical information from the applicant’s doctor
or hospital. The info will be faxed to Washington Home's admissions
office at (202) 895-0139.
Nurse managers and doctors, who make admissions determinations,
review an applicant’s medical information. If the applicant
is considered appropriate for The Washington Home’s Special
Care Alzheimer’s
Disease and Dementia Program, an interview is scheduled for the
applicant and the applicant’s family.
STEP TWO: Applicant
Interview
The interview includes a doctor, nurse manager and social worker
and other unit staff, who ensure that the applicant is appropriate
for
the program.
If the resident is considered appropriate for the unit,
The Washington Home Admissions Office extends an offer for admission.
The applicant
or the applicant’s representative accepts or rejects
the offer for admission. Financial issues are discussed with
the Admissions Director. Sources for payment include private
payment, long-term care
insurance, or D.C. Medicaid.
STEP THREE: Move In
The move-in date and time is scheduled by the Admissions Director
with the family.

The Admissions Office staff will work closely with area
insurance providers to secure the necessary authorizations for
admission
and will maintain
communication with the providers throughout your stay.
The staff will also discuss alternate methods of payment with
you
when
non-covered services are medically necessary.
