Hospices Program
At The Washington Home, hospice services are offered for those with
a terminal illness who are in the final phase of life. Patients often
choose our own Community Hospices program as their preferred provider
when selecting hospice care. Community
Hospices, one of the divisions
of The Washington Home organization, provides expert palliative care
and a wide range of other services to patients, enabling them to live
in their residence at The Washington Home and avoid unwanted hospital
transfers. People with a terminal illness - as a result of cancer,
Alzheimer's disease,
end-stage
heart disease, a debilitating stroke or other condition – can
benefit from hospice when curative treatment is no longer effective.
Community Hospices' Team
members include:
• Physicians
• Nurses
• Certified nursing assistants
• Home health aides
• Social workers
• Bereavement counselors
• Chaplains
• Volunteers
In addition to being experienced in symptom management,
the hospice team advocates for whatever is necessary and possible
to meet each
patient's
unique, end-of-life needs and wishes. Working in tandem with
the staff of The Washington Home, the Community Hospices' team will
provide hospice
services if they are clinically appropriate for the resident
and
requested.
