About The Washington Home & Community Hospices

The Washington Home & Community Hospices provides long-term, Alzheimer's, rehabilitation and hospice care in its facility at 3720 Upton Street, NW, and also provides hospice care to residents in their homes, assisted living facilities and hospitals in the District, Maryland and Virginia. Last year, The Washington Home & Community Hospices provided services to 2,507 residents and patients, including: 204 individuals who received residential, Alzheimer's disease and dementia care; 525 patients who received rehabilitation services after surgeries, traumas and illnesses; and 1,778 terminally ill patients who received hospice at-home care.
