Subacute/Rehabilitation In some cases, self-referrals are also made by patients who have scheduled surgery and know that they want to come to The Washington Home for rehabilitation. In addition, some patients, upon returning home following hospitalization, find that they need some rehabilitation to perform their daily tasks easily. They may request a doctor's referral. Doctors, discharge planners, or prospective patients or their families should call the Admissions Office at The Washington Home for more information.
