Hospice Care in Assisted Living Facilities

Assisted Living Facilities (ALFs) provide an intermediate level of care for people needing assistance with Activities of Daily Living (ADLs) who want to live as independently as possible for as long as possible. Residents might need assistance with such daily living tasks as eating, bathing, dressing, laundry, and housekeeping. Medical care is often available, but it is not as comprehensive as the care given in a nursing home

When residents of ALF's face terminal illnesses, Community Hospices provides hospice services just as we do for patients receiving hospice care at home.

ALF hospice patients receive:

  • patient care plans
  • interdisciplinary hospice care teams including:
    • Nurse Case Manager
    • Home Health Aide
    • Social Worker
    • Chaplain (if desired)
    • Volunteer (if desired)
  • medications required to treat pain management related to terminal illnesses
  • supplies and medical equipment necessary to treat terminal illnesses
  • therapies, including physical therapy, occupational therapy and speech therapy,as necessary to provide comfort to the patient
  • nutrition consultation as needed

In addition, our medical directors are available to answer patients' and families questions and to coordinate medical care with the patients' attending physicians to insure effective pain management.

Bereavement
As with all of our hospice patients, our comprehensive grief services, individual counseling, regularly scheduled support groups and other resources are available to families and loved ones.
